Diagnosis: Mycosis fungoides
Description: There is a superficial mid and deep dermal infiltrate of lymphocytes. In the mid and deep dermis the lymphocytes are predominantly perivascular although partly interstitial in location. These lymphocytes show mild nuclear atypia.

Differential DiagnosisHistory: 5349ma 73 years old lady with a 6 months history of these plaques under her breasts and on her abdomen. The colour varies. They are itchy. They never disappear despite a weak steroid cream. On several drugs but none new. Biopsies taken from 3 areas on the flank. DD early T cell lymphoma, Chronic superficial dermatitis, Fixed drug

Comments: There is a superficial mid and deep dermal infiltrate of lymphocytes. In the mid and deep dermis the lymphocytes are predominantly perivascular although partly interstitial in location. These lymphocytes show mild nuclear atypia. There is epidermotropism with focal infiltration of the lower half of the epidermis by the lymphocytes. The features present most suggest mycosis fungoides.
